This Social Worker opportuninty is full time @ 40 hours per week, ofers full benefits and a convenient dayshift schedule. This dynamic indibidual will be responsible to provide quality case management and psychotherapeutic services. To serve as a member of multi-disciplinary team supporting the hospital's treatment program and philosophy and assure the deliverance of quality treatment to patients and their family members. Provides social histories, discharge planning, case management, process and psycho-educational groups to age-specific populations.

Job Duties/Responsibilities:

  • Demonstrates ability, and is responsible to perform psychosocial assessments on Medicare, Medicaid and any high-risk patients to determine social service and possible discharge planning needs. Documentation of assessments and interventions is timely and meets current practice standards and policies.
  • Maintains current knowledge of resources available within the community; maintains supply of resource materials to be distributed to patients when needed. Is able to obtain other resources as needed.
  • Communicates with family members and caretakers regarding the needs of the patient and anticipated plans. Involves patient and family in the plan of care.

Qualifications

Social Worker:

  • Masters Social Work (MSW) preferred, BSW required.
  • Current state Social Worker licensure.
  • 3 or more years of previous experience in all aspects of social service and discharge planning preferably in an acute care setting.
