Department:ICHI | Community Programs 2Department's Website: Summary of Job Duties:The Nurse Educator will work within the identified Family Medicine Clinic to implement and promote the benefits and value of an Evidence-Based program to maintain a Patient and Family Centered Care approach. Develops a clinical strategy for Well Child Checks 0-5 years pre-visit planning and coordinates the transition of care for pediatric patients. Oversees Community Health Worker's call encounters to direct patient follow-up care. Effectively collaborates with nursing staff at all levels and participates with the clinic's complex care team to ensure children who are high-priority beneficiaries receive optimal care-bilingual preferred (Spanish, Marshallese). The nurse Educator will support the Family Medicine Residency by providing pediatric/Prenatal knowledge and skills through daily clinical encounters and participating in residency and nursing learning activities.
