REPORTS TO:

Direct Supervisor/Director of Advanced Practice Professional of the Heart and Vascular Institute

FLSA STATUS:

POSITION SUMMARY:Coordinates and manages the care of the CT surgery patient across the continuum of care. Works in collaboration with the CT Surgery providers to ensure comprehensive acute/ long-term care for patients managed by the service. Helps develop the team to meet quality and performance measures for CT Surgery. The Advanced Practice Professional (APP) coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient, and reviews the plan of care with the other members of the clinical team, participates in directing that care. The APP will order/perform diagnostic/therapeutic procedures, interpretation of such studies, will be involved with all aspects of theperi-operative services and coordinates discharge/follow up planning. The APP will work with the team to ensure adherence to protocol requirements and regulatory guidelines. They will ensure protocol compliance and close monitoring of order sets. The APP participates in the education of residents and medical/APP/nursing students.

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include, but are not limited to.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.Graduate of a CCNE accredited nursing program with Master's Degree in Nursing

2.National Certification by one of the recognized APRN Certification Centers

3.Licensure,or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse inWest Virginia.

4.BLS certification required. Additional life support certification at the discretion of the department.

5.Department Specific: APRNs requesting to beprivilegedas First Assists inperi-operative services will be required tocomplete an ANOR approved CNOR Certificate program and obtain a passing grade on CNOR certification exam.

6.Advanced Cardiac Life Support (ACLS) at the discretion of the department.

1.CT Surgery, Acute care or cardiology experience preferred.

MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.Bachelor'sORMaster's Degree as a Medical/Surgical (as applicable) Physician Assistant required.

2.Graduate of an ARC-PA approved Physician Assistant (PA) training program required.

3.Certification by the National Commission for Certification of Physician Assistants required.

4.Must be licensed or eligible for licensure by the WV Board of Medicine.

5.BLS certification required. Additional life support certification at the discretion of the department.

6.Advanced Cardiac Life Support (ACLS) at the discretion of the department.

EXPERIENCE:

1.Eligible new graduateswill be considered.

1.Manages comprehensive and individualized care for cardiothoracic surgery patients and their families in the inpatient phase of hospitalization and immediate post discharge in the ambulatory setting.

2.Completes daily rounds: consults and/or pre-procedure evaluations during weekdays and as applicable, weekends/holidays as determined by specific clinical site and specialty.

3.Completes integrated assessments, documents efficient and precise clinical evaluations/plan in the electronic medical record (EMR)

4.With the collaboration of faculty or by following established clinical Guidelines and Protocols, orders additional labs and/or diagnostic studies or consults as deemed appropriate

5.Performs, interprets and /or communicates results of diagnostic tests following established clinical guidelines and scope of practice in the inpatient and outpatient setting

6.Communicates topatientscomprehensive care team results of labs / diagnostic studies following established clinical guidelines and scope of practice, regarding abnormal findings in the evaluation of the patient.

7.Develops a comprehensive individualized plan of care for CT Surgery patients, which includes teaching, discharge planning and necessary referral to other members of the health care team as necessary.

8.Identifies unstable orcomplex patient care situationsand facilitates appropriate intervention with appropriate resources, in the clinic and/or hospital setting.

9.Recognizesemergency situationsand initiates effective emergency care.

10.Facilitates patient discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable).

11.Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.

12.Possess and/or acquire the appropriate surgical skill set to be able to first assist in all CT surgery operations performed by the service (as applicable)

13.Possess and/or acquire the appropriate surgical skill set to be able to perform open and/or endoscopic harvesting of appropriate conduit during the CT surgery operation(as applicable)

14.Assists in the collection of outcomemeasures (as applicable).

15.Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families.

16.Demonstrates the ability to utilize equipment in a safe manner.

17.Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings.

18.Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community.

19.Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn.

20.Updates own knowledge base throughout the year.

21.Completes all identified clinical competencies.

22.Serves as a resource person for staff caring for the CT surgery patient.

23.Participates and initiates inter-disciplinary approaches to patient care, program development and education. Serves as a professional rolemodel.

Additional Job Description:

This position is primarily a Thoracic Surgery position that will be responsible for inpatient, operating room (APRNs will be required to have their RNFA certification) and outpatient support. Occasional weekend may be considered.

Operates out of UHC. Clinic is located in the Bridgeport Office Building.
