The clinical pharmacist II supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates proficiency in all basic clinical pharmacist skills. The clinical pharmacist II must have met all general requirements of the clinical pharmacist position, in addition to requirements as set forth by the department Career Ladder.

Responsibilities
- 1. Demonstrate competency in pharmacy automation and technology as applicable to work area
- 2. Oversee sterile and non-sterile compounding to ensure compliance with regulatory statutes (e.g. USP
Chapters), as applicable to work area - 3. Oversee medication distribution/dispensing process in work area, to ensure safe, effective, timely
medication distribution/dispensing - 4. Safely and correctly compound sterile medications and infusions for the home, outpatient clinics, skilled
nursing facilities and physician/clinic environments. - 5. Initiate infusion therapies with successful on-going evaluation of care and completion of required
documentation relative to processing therapies, both fiscal and legal. - 6. Responsible for understanding of, compliance with and participation in Quality Assurance, Performance
Improvement and accreditation guidelines ensuring achievement of excellence in quality patient care. - 7. Assist with the referral process including but not limited to: provision of medical information to patients and
caregivers relative to home infusion therapy; assistance in the teaching of patients prior to discharge to home ; and coordination of deliveries with nursing and support services. - 8. Be actively involved in the education and inservicing of licensed health care professionals who perform
services relative to the therapies provided by the home infusion center. - 9. Maintain working knowledge of all equipment and supplies necessary to administer therapies in the home or
outpatient setting. - 10. Responsible for knowing and adhering to all home infusion pharmacy Policy and Procedures.
- 11. Participate in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance
and hazardous waste control. - 12. Responsible for emergency on-call duties on weekends and weeknights as required by center scheduling.
- 13. Learn and utilize all computer programs necessary for the successful review, authorization, preparation,
dispensing and management of home infusion pharmacy patients. - 14. Remain familiar with literature, references and resources for therapies and care of patients serviced by the
home infusion center. - 15. Maintains the confidentiality of patients and their personal information at all times.
Qualifications
EDUCATION Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from a college or school of pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) or equivalent required. EXPERIENCE Three years of relevant pharmacy experience. Completion of an American Society of Health-System Pharmacists (ASHP) accredited PGY1 pharmacy residency program, and if available, a PGY2 program in an area relevant to the position are strongly preferred and may completely or partially fulfill the above experience requirements. LICENSURE Eligibility for pharmacist licensure in the state(s) as determined by the scope of the position is required. Board certification is preferred if certification relevant to the position is available. SPECIAL SKILLS This individual should demonstrate in depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. PHYSICAL DEMAND Working in a cleanroom space, gowning, garbing and handwashing. This requires handwashing prior to compounding and post compounding when leaving the cleanroom. Employee may be expected to work on -call shifts. Coverage extends to day, and evening shifts. Employee is considered essential personnel and is expected to work during inclement weather (i.e.blizzards, hurricanes) if scheduled. During to course or daily tasks, the employee is required to walk, sit, stand and crouch for extended periods of time. The employee must regularly lift/move up to 10 pounds, frequently lift/move up to 25 pounds and occasionaly lift/move up to 50 pounds. The employee is occasionally exposed to moving mechanical parts, fumes airborne particles, and toxic or caustic chemicals. The work environment noise level is moderate.
