The Blood and Marrow Transplant (BMT) and Cellular Therapy Clinical Social Worker serves as an integral member of a highly specialized multidisciplinary team caring for patients undergoing hematopoietic stem cell transplantation and cellular therapies. This role provides comprehensive psychosocial assessment, counseling, care coordination, caregiver support, and resource navigation to patients and families facing complex, life-threatening illnesses throughout all phases of treatment and recovery including end-of-life.
The successful candidate will build therapeutic relationships with patients and caregivers, conduct psychosocial evaluations and risk assessments, identify barriers to treatment, facilitate discharge and transition planning, connect families with community and financial resources, and provide evidence-based interventions that promote coping, resilience, and quality of life. The social worker collaborates closely with physicians, advanced practice providers, nurses, care coordinators, and community partners to ensure safe, patient-centered care and optimize treatment outcomes. This position requires advanced clinical judgment, strong interdisciplinary collaboration skills, and expertise in addressing the emotional, social, financial, and practical challenges associated with transplant and cellular therapy. The role also contributes to program development, caregiver support initiatives, patient education, advocacy efforts, and continuous improvement activities that advance comprehensive psychosocial care within the transplant program. The purpose of the Clinical Social Worker 3 (MSW) position is to provide both direct patient care to patients and families and assist in the coordination of team delivered services. The Clinical Social Worker 3 will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities. Licensure, Certification, or Registration Requirements for Hire: Eligible LCSW team members may provide clinical supervision to Social Work Supervisees pursuing licensure, on a voluntary basis. Eligible MSW team members may provide field instruction to current MSW students, on a voluntary basis.
