Substance Use Disorder Professional (SUDP) - Case Manager
Telecare FPATH #269 | Washington Official Telecare Job Title: Case Manager Substance Use Counselor Certified Help People Build Recovery Where It Matters Most - In Their Community Telecare's FPATH #269 is seeking a Washington State Substance Use Disorder Professional (SUDP) who wants to do meaningful, hands-on work with adults living with serious mental illness and co-occurring substance use challenges. This is not a traditional office-based counseling position. You will work directly with members in the community, helping them address substance use, behavioral health needs, housing, healthcare, daily living needs, and other barriers that can impact long-term stability. Using a harm-reduction, recovery-focused approach, you will meet people where they are, build relationships over time, and help members identify and work toward recovery goals that are meaningful to them. About FPATH #269 FPATH is a community-based behavioral health program serving adults living with serious mental illness, including individuals experiencing co-occurring substance use challenges and complex barriers to stability. Rather than expecting members to navigate recovery alone or receive all services in a traditional office setting, our team brings support directly into the community. Our multidisciplinary team works together to address the whole person-including behavioral health, substance use, physical health, housing, daily living skills, relationships, and connection to community resources.
