Job Summary As a Clinical Dietitian, you will play a key role in improving patient health by delivering evidence-based medical nutrition therapy across a diverse patient population. Working collaboratively with physicians, nurses, and other healthcare professionals, you will assess patients' nutritional needs, develop individualized care plans, provide nutrition education, and help support positive clinical outcomes.Essential Functions -Assess patients' nutritional status using clinical data, physician orders, interdisciplinary care plans, and evidence-based practice to develop individualized nutrition care plans. -Provide medical nutrition therapy, monitor patient progress, and adjust nutrition interventions as appropriate throughout the course of care. -Document nutrition assessments, recommendations, care plans, follow-up evaluations, discharge planning, and other nutrition-related activities in the electronic medical record. -Educate patients and their families on therapeutic diets, nutrition goals, and strategies to support long-term health and wellness. -Collaborate with physicians, nurses, and other members of the care team to ensure coordinated, patient-centered nutrition care. -Partner with food service teams to help ensure patients receive appropriate meals, supplements, and nutrition support while promoting adherence to prescribed diets. -Develop and maintain educational resources to support patient and family nutrition education.
