Under general supervision and the direction of the Medical Director, the Nurse Coordinator provides comprehensive care coordination for patients within the Pulmonary Vascular Program, with a focus on pulmonary arterial hypertension (PAH). This role supports a rapidly growing and complex patient population through clinical care, program coordination, and patient education.

Key duties include: coordinate care for patients with pulmonary arterial hypertension, including complex cases requiring advanced therapies (e.g., prostacyclin infusion), perform patient assessments and assist with diagnostic and treatment planning, review laboratory and diagnostic results; provide follow-up recommendations and patient communication, manage medication refills, prior authorizations, and insurance justification processes, triage incoming patient calls and address clinical concerns in a timely manner, coordinate procedures, referrals, and continuity of care, provide comprehensive patient and family education, and serve as a liaison between patients, families, and the interdisciplinary care team.

The Nurse Coordinator practices autonomous nursing care, utilizing professional expertise, clinical judgment, and critical thinking consistent with the American Nurses Association (ANA) Standards of Practice and Professional Performance. This position functions within a multidisciplinary team and collaborates closely with the senior pulmonary arterial hypertension nurse coordinator, attending physicians, fellows, advanced practice providers, and administrative staff to ensure high-quality, patient-centered care.

• Must be a graduate from an accredited school of Nursing.

• RN license issued by the State of California.

• BLS and ACLS certifications at time of hire with commitment to get UCSD ART within six (6) months of hire date.

• Minimum of five (5) years of active RN experience.

• Minimum of two (2) years of recent outpatient clinic experience (within the last five years).

• Minimum of two (2) years recent pulmonary vascular experience (within the last five years).

• Minimum of one (1) year experience in patient triage, in-basket management, and appointment scheduling.

• Demonstrated knowledge of pulmonary arterial hypertension (PAH).

• Knowledge of EPIC/Cadence, including obtaining insurance authorizations.

• Demonstrated ability to maintain accurate and complete patient records.

• Knowledge of various insurance plans and the authorization processes.

• Knowledge of medical terminology, preferably related to pulmonary medicine.

• Ability to work effectively in a fast-paced environment with excellent interpersonal, verbal, and written skills, while consistently demonstrating empathy and compassion toward patients and colleagues.

• Demonstrated critical thinking and clinical judgment skills to assess patient needs, implement appropriate nursing interventions, and evaluate outcomes.

• Experience providing, or demonstrated interest in providing, individualized care to vulnerable populations.

• Ability to communicate clearly and effectively, both verbally and in writing.

• Bachelor of Science in Nursing (BSN).
• Bilingual in English and Spanish.
• Experience caring for patients with pulmonary arterial hypertension (PAH).
• Experience using EPIC electronic health record (EHR) systems.

• Must be able to work various hours, days, shifts, on-call and various locations based on the 24-hour Medical Center's business needs.
