Location: Remote/Virtual - Must Live and Work in Michigan

In-State Travel and Occasional In-Person Meeting Attendance Required



Purpose: This position will provide consultation, technical assistance, and contract monitoring, per Michigan Department of Health & Human Services (MDHHS) requirements, to local evidence-based home visiting programs primarily in Western Michigan, funded through the Home Visiting Section. This position will help to ensure contract expectations are met through the provision of high-quality home visiting services to eligible pregnant women and families with children. This position will also act as a support for programs to assist with program implementation as it pertains to contract requirements. The Home Visiting Model Consultant will work with the Michigan Home Visiting Section (HVS) within MDHHS, serving as an integral member of the state team, regarding specific prevention-focused home visiting models for pregnant women and families with young children. This position will also work collaboratively with state and local partners to support a comprehensive home visiting system.



Duties and Responsibilities:

  • Provide consultation and technical assistance to local grantees implementing home visiting programs in their communities.
  • Establish contract language for local implementing programs.
  • Work collaboratively with HVS fiscal staff to ensure appropriate sub-recipient monitoring of the Local Implementing Agency (LIA).
  • Support LIAs through meetings with individual programs, programs in specific communities or regions, or by model, as needed, but no less than monthly.
  • Prepare and utilize all HVS sub-recipient monitoring documents, with HVS fiscal staff as appropriate, to complete required sub-recipient monitoring activities.
  • Participate in all appropriate LIA programmatic site visits.
  • Work with HVS program staff to analyze and develop procedures to integrate evidence-based home visiting into the broader Early Childhood and Home Visiting Systems.
  • Collaborate with key MDHHS and Michigan Public Health Institute staff to evaluate program measures, monitor the effectiveness of interventions, and support Continuous Quality Improvement efforts.
  • Participate in conversations with the National model representatives to ensure a strong collaborative relationship.
  • Participate in grant development/writing as needed, conference planning, and other MHVI activities as identified by the manager.
  • Participate in national and state advisories as requested or invited.
  • Contribute to the overall planning and implementation of the state's Home Visiting system.


  • Maintain a positive, credible, professional, and interpersonal relationship with all state and local partners and always represent the best interest of MDHHS/MPHI.
  • Participate in HVS Implementation Plan Review meetings as part of grantee monitoring.
  • Align activities with the HVS Child Welfare Coordinator and Substance Use Coordinator to support implementation of Thriving Futures.
  • Participate in section, division, bureau, and department level activities that support personal development and growth in knowledge on racism, diversity, health inequities, and health disparities.
  • Understand current section, division, and department activities to reduce health disparities and increase overall diversity in the workforce.
  • Support inclusion of representative parent voice as an equity strategy in all state and local grantee activities.
  • Participate in all required state/MPHI training.
  • Develop policies and procedures for grantees to support practices that include consideration of unique community variables and consider impact of those variables prior to implementation.
  • Always represent the best interests of MDHHS and MPHI.
  • All other duties as assigned.



Qualifications/Requirements:

Education: Master's degree in nursing, public health, or a related field.



Experience: One year of experience in maternal and/or child health required. Three years' professional experience in maternal and/or child health preferred.

One year's experience in prevention focused evidence-based home visiting required. Three years of professional experience in prevention-focused home visiting preferred.

Experience providing leadership to other professionals or community groups via consultation, training, supervision, or administration is required.

  • Techniques of individual and group collaboration.
  • Knowledge of prevention-focused home visiting programs for pregnant people and families with young children.
  • Ability to provide leadership to other home visiting professionals.
  • Ability to gather feedback from stakeholders and identify training needs.
  • Support access to training; and plan and facilitate meetings.



Work Environment and Physical Requirements: Remote work is standard. Attendance at in-person Department (Bureau, Division, Section) meetings is required. Involves viewing a standard screen a minimum of 75% of the time. Requires travel to designated communities across Michigan, as needed. Valid vehicle operator's license needed. May be required to travel to national meetings relevant to the implementation of the project and/or state sponsored activities supporting the project.

IMPACT ON PROJECTS, SERVICES AND OPERATIONS: This position has a direct impact on the quality of services provided by MPHI to MDHHS. This position will have a direct impact on the quality implementation of evidence-based home visiting programs implemented primarily in Western Michigan, ultimately improving outcomes for families with high-risk, as well as impacting implementation of home visiting statewide.
