Our Case Manager/Service Coordinator RN will perform case management and service coordination activities that enable Cook Children's Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation. This is a remote based position with required field work. Requires driving and in-home visits with children and families up to 75% of the time (mileage reimbursed). Some weekends may be required to meet member needs.Hours: Full-time days, some weekends may be required. Must be able to travel to members' homes within Tarrant, Wise, Denton, Parker, Hood and Johnson County.

Required Education, Experience & Skills:

• Associates Degree in nursing required, BSN preferred.

• Three (3) years of clinical experience.

• Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions preferred.

• Strong skills in medical assessment/medical record review and/or behavioral health assessment.

• Prior MCO experience a plus.

• Strong Relationship-building skills with effective written and verbal communication skills that allow clear and concise presentation of conclusions and recommendations to a variety of audiences.

• Critical thinking and problem-solving skills with the ability to plan and organize.

• Self-starter with the ability to be effective in a large, complex and highly matrixed organization is a must.

• Must possess the ability to function in an interdisciplinary, team environment.

• The ability to focus on both the details and the big picture simultaneously.

• Proficient skills in computer applications (Excel and Outlook).

• Bi-lingual (English/Spanish) Preferred.

Certification/Licensure:

• Registered Nurse current, unrestricted Texas license.

• Must possess a valid Texas Driver's License and have own vehicle.
