Responsibilities
Qualified Mental health Professional- OTP The Qualified Mental Health Professional will provide crisis intervention and clinical support to individuals involved in diversion and re-entry programs who are incarcerated or recently released. The primary goal is to stabilize mental health crises, assess client needs, and facilitate connections to appropriate community resources and treatment services. The position requires collaboration with correctional facilities, community partners, and healthcare providers to support successful reintegration and reduce recidivism. The Qualified Mental Health Professional (QP) will provide crisis response support, linkage to outside agencies to whom participants are referred, care coordination, and connection to Emergency Departments and the Behavioral Health Urgent Care Center. The Guilford County Sheriff's Office Re-entry Program and Jail Diversion Program serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The Guilford County Sheriff's Office, in collaboration with Foundations Health High Point will provide Medication Assisted Treatment (MAT) services for individuals detained in the Guilford County Detention Center who have a Substance Use Disorder (SUD). This position is supported by grant funding for an initial 18 month period, with the opportunity to extend for another 12 months. Following the grant period, there may be opportunities for the role to continue as part of ongoing operations, contingent on program sustainability and organizational needs.
