ADDENDUM FOR FORMER CLINICAL S Clinical Triage and Intak Coordinator A Clinical Triage and Intake Coordinator is a frontline healthcare professional responsible forreceiving new patient referrals, conducting initial clinical assessments, and determining the appropriate level of care. They verify insurance, manage Electronic Health Records (EHR), and coordinate with providers to ensure prompt, accurate patient placement and care. Key Responsibilities *Clinical Assessment and Triage: Evaluates patient symptoms and medical history to determine urgency and appropriate care, such as directing mobile crises, PHP/IOP, interventional psychiatry, medication management, outpatient therapy for individual and/or therapeutic groups. *Communication & Liaising: Acts as a primary contact for patients, families, and referral sources to foster positive relationships and ensure smooth, and compliant patient onboarding. Managing inquiries via fax, phone, email, and EHR. *Intake Coordination:Manages waitlists and the full admission process, including gathering patient information, verifying insurance coverage, obtaining necessary authorizations, confirming patient financial responsibility and scheduling initial behavioral health appointments. *Clinical Diagnostics: Perform initial diagnostic evaluation and formulate preliminary treatment plans, as needed. *Documentation & EHR Management:Accurately records assessments, patient history, legal status, and insurance information within the Electronic Health Record (EHR) system.Ensuring all forms are accurate, signed, and maintained in compliance with HIPAA regulations. *Care Plan Assistance:Collaborates with medical staff to set patient goals and coordinate appropriate behavioral health programming. *Community outreach and Education: Facilitates education on programming and referral process.
