Social Worker - Emergency Department (2:30 p.m. - 11:00 p.m.) Summary The Social Worker I provides direct psychosocial care, discharge planning, and care coordination for Emergency Department patients from 2:30 p.m. to 11:00 p.m., collaborating with patients, families, physicians, nurse case managers, and the interdisciplinary team. Services include psychosocial assessment, goal setting, behavioral interventions, crisis intervention, patient/family education, and referral to community resources to mitigate barriers to care and discharge. Key Responsibilities
- Provide clinical social work services in the Emergency Department, working closely with nurse case management and interdisciplinary teams.
- Screen patients and complete full psychosocial assessments and individualized care plans.
- Deliver psychosocial interventions for behavioral health, substance use, adjustment to illness/hospitalization, crisis situations, and post-hospital planning.
- Conduct family meetings, facilitate care transitions, and coordinate referrals to community resources.
- Provide psychotherapy, crisis intervention, and monitor medication effects when applicable (Community Psychiatry Program duties).
- Coordinate care with the multidisciplinary team and participate in interdisciplinary rounds as appropriate.
- Document assessments, plans, and interventions in the medical record according to departmental and regulatory standards.
- Maintain and utilize a network of local, regional, and national community resources; assist with legal guardianships, psychiatric referrals, abuse/neglect reporting, end-of-life and charity care as needed.
- Participate in quality improvement, committee work, supervision per COMAR, on-call coverage, and occasional home visits.
- Provide coverage during staff absences; other duties as assigned.
Qualifications
- MSW required.
- Maryland LMSW required.
- Experience in a medical setting required.
- Strong interpersonal and communication skills; ability to work collaboratively in an interdisciplinary team.
- Knowledge of community resources and ability to synthesize assessment data to guide care.
- Ability to prioritize, manage a full caseload, and document promptly.
- Proficiency with electronic health records, email, and standard office technology.

Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
