Adult Health Home Team Leader

The Adult Health Home Team Leader will provide supervision and day to day operations of the adult health home program. The Team Leader will work on a Full-Time basis in the Syracuse area.

Pay Rate: $62,350/annual

Maintains and provides supervision of program staff.
• Carry a small caseload.
• Participates in Management and Administrative committees as assigned.
• Assures program record documentation is in compliance with agency and regulatory standards.
• Represents agency to the community.
• Other duties as assigned by supervisor.

• Minimum three years of health home experience in an appropriate program setting.
• Minimum Bachelor's Degree, Master's Degree in a related human service field strongly preferred.
• Previous supervisory experience preferred.
• Must have a valid driver's license and own transportation:
  • Helio Health's insurance carrier requires that employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date on Declaration Page.
