Job Title: Home Health Physical Therapist

Job Summary:
We are seeking a compassionate and skilled Home Health Physical Therapist to provide high-quality, patient-centered care in a home setting. In this role, you will help patients improve mobility, regain independence, and enhance their overall quality of life through personalized therapy plans.

Key Responsibilities:

• Evaluate patients' physical condition, strength, and mobility to develop individualized treatment plans
• Provide prescribed physical therapy services to patients with injuries, illnesses, or functional limitations
• Design and implement rehabilitation programs in alignment with physician orders and patient goals
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
• Collaborate with physicians and interdisciplinary care teams to ensure coordinated care
• Supervise and support Physical Therapy Assistants and Home Health Aides, ensuring adherence to care plans
• Maintain accurate, timely documentation of patient care, progress notes, and treatment updates
• Communicate effectively with patients, families, and healthcare team members regarding care plans and progress
• Submit evaluations, progress reports, and discharge summaries in a timely manner
