Performs job responsibilities of a Program Associate at designated program sites and serves as a medical liaison with healthcare providers, program management, etc., on behalf of assigned person(s) served.



Responsibilities

In addition to performing job responsibilities of a Program Associate, serves as a liaison between
assigned persons served and their healthcare and insurance providers in advocating the best possible
medical treatment for persons served:

1. Schedules individual medical appointments for person(s) served in accordance with healthcare
providers' orders, Interdisciplinary Team (IDT) recommendations, and in compliance with Division
of Developmental Disabilities (DDD) regulations and requirements as directed from the Health Care
Coordinator ;
2. Prepares assigned person(s) served for all medical appointments with physicians, dentists, and
healthcare specialists; e.g., washing, bathing, grooming, appropriate attire, etc.;
o Ensures the accuracy, completeness, and currency of all medical information maintained at the
program site for assigned person(s) served in their caseload as directed by the Health Care
Coordinator;
3. Prepares for individual medical appointments for assigned person(s) served; e.g., confirms
insurance and funding information prior to visit, ensures that all necessary pre-testing has been
performed and results received; e.g., laboratory tests, x-rays, special studies, etc.;
4. Personally accompanies assigned person(s) served to medical visits or makes arrangements for a
proxy; communicates with healthcare providers on behalf of person(s) served, providing medical
history, explaining current health status, behaviors and reason for visit, providing details regarding
medications and reactions, etc.;
5. Performs necessary follow-up post-medical visits; alerts program site staff of any medication
changes; observes and monitors person(s) served post-administration of newly-prescribed drugs
and promptly notifies healthcare providers and program management in the event of an adverse
reaction; appropriately updates and files medical information for assigned person(s) served;

6. Provides supervision and direct care to assigned person(s) served in the event of medical or dental
procedures, etc., which may vary from short duration to extended periods of time or until relieved;
7. Serves as liaison with healthcare providers, insurance providers, equipment suppliers, etc., in
arranging for and acquiring various medical equipment and supplies for assigned person(s) served;
8. Serves as an advocate on behalf of assigned person(s) served in a variety of situations.



Qualifications

Minimum Requirements:

Must be 18 years of age

High School Diploma or GED

Valid Driver's License in the candidates' legal name and current address. One year minimum

driving experience. License should be in good standing and free of restrictions.

Minimum of six months related work experience OR Bachelor's degree or current enrollment in a degree program in Psychology, Social Work, or related field

Desired skills:

Aspiration to work with autistic, intellectually and developmentally challenged individuals

Prior experience working with individuals with autism, intellectual or developmental disabilities preferred

Effective verbal and written communication skills
