A Brief Overview

The Coach Navigator serves as a bridge between the community, health care and the social service systems. The Coach Navigator acts as a liaison by helping adolescents develop their capacity and access to resources. They work in both the clinical research setting and community-based setting. The Coach Navigator provides support by making hospital and/or and community-based visits to educate adolescents on accessing quality care and health information.

What you will do

• Work to reduce cultural and socio-economic barriers between adolescents and institutions
• Refer adolescents to community resources, social services, insurance providers and other relevant providers for medically complex patient and families recommended or identified by the care team
• Continuously expands knowledge and understanding of community resources and services
• Provide clear and concise documentation of adolescents interactions and visits
• Motivate adolescents to be active and engaged participants in their health and overall wellbeing
• Engages adolescents in obtaining self-management skills for management of health needs
• Utilizes high-level engagement techniques to establish rapport with adolescents
• Contributes to the orientation and on-boarding process of new staff

Education Qualifications

• High School Diploma / GED Required

Experience Qualifications

• At least three (3) years of community-based health work, working in the pediatric community, care coordination or case management Required

Skills and Abilities

• Ability to travel by car or independence on public transit, taxi, or ride-sharing program
• Knowledge of local community agencies and organizations.
• Strong interpersonal skills, communications skills, problem-solving skills, teaching and coaching skills, and community networking skills.
• Ability to work effectively with a wide range of constituencies in a diverse community.
• Personal experience with medical complexity preferred but not required
• Outstanding customer service and communication skills
• Ability to work independently and in collaboration with other teams
• Ability to navigate Smartphones and mobile devices; IOS and Android
• Excellent computer skills; email, Word, EPIC
• Understands and values the importance of equitable access to healthcare
