Overview

Find Your Passion and Purpose as a CARE Continuum Liaison, serving as a trusted guide for seriously ill and chronically ill patients and their loved ones.

As a CARE Continuum Liaison, you will work within the Care Continuum to support seriously ill and chronically ill patients and their families as they navigate complex healthcare decisions and transitions. In this role, you will serve as a trusted guide, helping individuals understand their care options, identify appropriate additional levels of care, and move through critical moments with compassion, clarity, and confidence. You will collaborate closely with clinical teams, community partners, and internal stakeholders to help ensure patients receive the right support at the right time.

This position is ideal for someone who is passionate about guiding seriously ill and chronically ill patients through some of life's most difficult moments and helping them access the care they need across the continuum. Success in this role requires strong clinical judgment, exceptional communication, and a patient-centered approach to building trust, removing barriers, and supporting seamless transitions to services such as home health, hospice, palliative care, or other appropriate care settings.

Offer Based on Years of Experience

Salary range:$85,000-$95,000/yr.

Hospice experience Required

Home Health experience Preferred

SW, LPN/LVN or RN License in MN required for consideration

This is a field-based position supporting the Twin Cities, Minnesota market, working within the Care Continuum as a trusted guide for our patients

Hybrid role - work from home up to 3 days per week, with up to 2 days per week spent in the office or in the field supporting patients with transitions to additional levels of care

Ideal candidates are passionate about working within the Care Continuum as a trusted guide for our patients, especially during some of their most difficult moments. This role helps patients and families understand their options, navigate additional levels of care, and move through transitions with clarity, compassion, and confidence while partnering closely with care teams to support a seamless care experience.

  • Current licensed RN/APRN/MSW/LPN
  • Minimum of 5 years of direct clinical care experience in community-based setting.
  • Knowledge of CBT, motivational Interviewing, mindfulness, and solution-focused clinical interventions
  • Excellent listening, written and verbal communication skills.
  • Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.
  • Advanced organizational skills, self-starter, proactive, creative problem-solving skills.
  • Subject matter expert on advanced illness/serious illness care, home health and hospice benefit structures, and evidence-based clinical practice.
