Overview
Find Your Passion and Purpose as a CARE Continuum Liaison, serving as a trusted guide for seriously ill and chronically ill patients and their loved ones. As a CARE Continuum Liaison, you will work within the Care Continuum to support seriously ill and chronically ill patients and their families as they navigate complex healthcare decisions and transitions. In this role, you will serve as a trusted guide, helping individuals understand their care options, identify appropriate additional levels of care, and move through critical moments with compassion, clarity, and confidence. You will collaborate closely with clinical teams, community partners, and internal stakeholders to help ensure patients receive the right support at the right time. This position is ideal for someone who is passionate about guiding seriously ill and chronically ill patients through some of life's most difficult moments and helping them access the care they need across the continuum. Offer Based on Years of Experience Salary range:$85,000-$95,000/yr.
