New Hanover County Health Department is seeking a Health Educator to coordinate two state grant funded public health initiatives: the Hepatitis C Bridge Counselor program and the Integrated Targeted Testing Services (ITTS) program. The Hepatitis C Bridge Counselor program aims to rapidly identify clients with Hepatitis C within Region 8 (Bladen, Brunswick, Columbus, Duplin, New Hanover, Onslow, Pender, Robeson, and Sampson counties) and to link them to clinical providers for viral hepatitis care and treatment. The ITTS Coordinator partners with community organizations within New Hanover County to provide counseling, testing, and referral services for HIV, Syphilis, Hepatitis C, Chlamydia, and Gonorrhea to high-risk key priority groups in community settings. The health educator will work within the Community and Clinical teams to reduce community transmission of STIs, ensuring services are delivered in a culturally competent and confidential manner.
Essential Duties include:
- Facilitate rapid disease intervention by engaging newly diagnosed and referred clients in care for treatment.
- Providing ongoing support and monitoring for each Hepatitis C counselling referral.
- Provide education on and promote the testing and treatment of STIs.
- Establish and maintain effective working relationships with community organizations working with high-risk populations.
- Plan and implement community based STI testing.
- Document interactions in NCEDSS and Electronic Medical Record.
- Collect data and complete quarterly reports.
- Other duties as assigned.

Hours:
8:00 AM - 5:00 PM Monday through Friday. All position assignments must be available to work during emergency situations.
Salary Information:
The
hiring range for this position is
$60,943 - $80,764.50.
Salary will be determined based on qualifications.
Bilingual candidates are eligible for a differential (5%) when performing Spanish language interpretation during the course of work.
