The Adult Congenital Heart Disease (ACHD) Advanced Practice Provider (APP) at Akron Children's Hospital provides advanced outpatient-focused clinical care to adults with congenital heart disease. The APP works collaboratively with both pediatric cardiologists and ACHD cardiologists, Nurse Program Coordinator, and multidisciplinary teams to deliver comprehensive evaluations, ongoing management, patient education, and care coordination in an exclusively outpatient setting, with limited inpatient consult support as needed. Candidates must have at least two years' experience with adult chronic disease management. Essential Responsibilities Qualifications Schedule & Expectations - Conduct comprehensive and focused cardiovascular histories and physical examinations for ACHD patients. - Review prior medical records, imaging, and diagnostic studies prior to clinic encounters. - Develop assessments and management plans and collaborate with supervising pediatric cardiologists and ACHD cardiologists. - Order and interpret laboratory tests, diagnostic studies, and cardiac imaging. - Write detailed clinic notes and formal patient letters summarizing visits and recommendations. - Provide follow-up communication regarding testing, medication changes, recommendations, and ongoing symptom review. - Manage patient calls, triage concerns, and offer ACHD-specific guidance in collaboration with the nurse program coordinator. - Receive and respond to critical lab values with attending cardiologist oversight. - Assist with insurance prior authorizations and peer-to-peer reviews when required. - Prepare forms, including cardiac clearance, SBE prophylaxis, work/school letters, and letters of medical necessity. - Provide occasional inpatient consult follow-up when outpatient schedule permits. - Communicate clinically relevant inpatient updates to the ACHD outpatient team. - Serve as a resource for inpatient teams seeking ACHD guidance. - Coordinate care with primary care providers, referring cardiologists, and external specialists. - Support transition processes for young adults transferring into ACHD care. - Provide comprehensive patient and family education regarding congenital heart disease and long-term management. - Participate in ACHD clinical research initiatives including patient recruitment and data collection. - Collaborate with investigators on publications and conference presentations. - Engage in APP-led quality improvement projects aligned with ACHD program goals. - Precept APP and medical students in the ACHD outpatient setting. - Participate in onboarding and orientation for new clinic providers and staff. - Maintain required continuing education and remain current with ACHD clinical guidelines. - Current licensure as a Physician Assistant or Nurse Practitioner (FNP, AGPCNP, or AGACNP). - National certification (PA-C, FNP-C, or equivalent). - DEA license (or eligibility). - BLS and ACLS certification. - Experience in cardiology, adult congenital heart disease, or outpatient specialty care preferred. - Primarily outpatient-focused role with structured clinic sessions. - Minimal inpatient support responsibilities. - Occasional participation in outreach clinics or educational events. | |
