POSITION SUMMARY: The Population Health Nurse for the BHEAT Program supports the Patient-Centered Outcomes Research Institute (PCORI) research grant and the Remote Blood Pressure Monitoring (RBPM) initiative at GRMDC by serving as the primary clinical point of contact for enrolled patients. Working under the supervision of the Senior Manager of Value-Based Care and Nurse Manager, and in close collaboration with the Site Principal Investigator (PI), project team, and VBC Population Health Nurse, this role ensures that patient data is accurately captured and integrated into the electronic health record (EHR) in alignment with research study protocols. This role will serve as an extension of primary care clinicians, supporting the management of patients with hypertension. Through high-quality care coordination, data accuracy, and patient engagement, the Population Health Nurse advances GRMDC's mission to improve health outcomes and support innovation in population health and value-based care delivery. Compensation Range: $31.97- $46.39This range offers an estimate based on the minimum job qualifications.
