Responsibilities

Perform patient evaluations and develop personalized treatment plans.

Regularly reassess treatment effectiveness and participate in staffing meetings and family conferences.
Conduct discharge evaluations, recommend home and community safety modifications, and provide education and referrals.
Communicate patient progress and needs to the treatment team, physicians, patients, and families.
Collaborate with other disciplines to coordinate care and enhance patient outcomes.

Qualifications

Degree in Physical Therapy from an accredited program.
Active Physical Therapist license in the practicing state.
CPR or Basic Life Support certification per state regulations.
At least one year of physical therapy experience preferred.
Knowledge of rehabilitation techniques for complex neurological injuries.
Strong verbal and written communication skills.
Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.
Health screenings or exams may be required per location or state policies.
