Requirements

The ideal candidate will possess the following qualifications:



  • Bachelor's degree in social work or a human service-related field.
  • 5 years minimum experience in social work/case management is required.
  • Strong computer skills (Microsoft Word and Excel; SPSS; email and employee portal, teletime) and organizational skills.
  • Must have problem-solving, interpersonal Skills, and multitasking skills
  • Able to manage stress appropriately
  • Able to make decisions under pressure
  • Manage anger/fear/hostility/violence of others appropriately
  • Able to work alone and work in areas that are confined and/ or crowded areas

We are seeking a Case Manager to join our Riverview Recovery Wellness Program in Philadelphia, Pa.



This posting is full-time, Monday - Friday 4:00p - 12:00a.

Position Details:


This is a direct care position working with adult men and/or women veterans in the early stages of their recovery from chemical or alcohol dependency, co-occurring disorders, serious mental illness, and homelessness. You will be providing case management services to these men and/or women in a transitional housing setting and reporting directly to the program manager or director



Additional responsibilities will include:



  • Manage the needs of program residents.
  • Link residents to various community-based services and resources as needed.
  • Lead a case management team of supportive service coordinators and facility attendants and make recommendations to the program director.
  • Ensure all progress reports are completed properly and on time.
  • Incident reporting to include writing incident reports, reporting to the immediate supervisor, and following procedure as stated in Merakey policy.
  • Encourage and verify that residents with income are saving money.
  • Data input and outcome analysis.
  • Provide training as needed and directed by the program director.
  • Work with the program director to develop and enforce policy and procedure.
  • Life Skills Group facilitation (i.e., money management, housing acquisition, nutrition).
  • This position may also be asked to assist with interviews, intakes, and various other tasks in an effort to support program operations and the program director.
