Position Summary:

The Medication History Technician assists the clinical pharmacists in caring for patients by providing point of care services as assigned. Activities include assisting with patient's medication management and obtaining and organizing information such as patient demographics, medication histories and information needed for therapeutic drug monitoring. Promotes the safe and effective use of medications by providing the pharmacist with accurate and timely information necessary to make informed clinical recommendations. Works collaboratively with the multidisciplinary team and outpatient pharmacy services to reconcile medication history and/or facilitate discharge prescriptions. Participates in performance improvement activities to increase the scope and quality of pharmacy services. Works with the pharmacy leadership team to ensure the cost-effective utilization of medications.

Shift: Full-time, Evenings, Weekends Required

Education:

High school education or equivalent is required.

Licensure/Certification:



  • Registered in the State of Maryland as a Pharmacy Technician.
  • National certification from a Maryland Board of Pharmacy approved program: Exam for the Certification of Pharmacy Technicians (ExCPT), Institute for the Certification of Pharmacy Technicians (ICPT), National Health Career Association (NHA), or Pharmacy Technician Certification Board (PTCB) within 90 days of hire.


Required Work Experience:



  • Two years of hospital and/or community Pharmacy Technician experience is preferred.
  • Completion of pharmacy provided orientation and training and demonstrated competence in knowledge, skills, and abilities is required within sixty (60) days of hire date under minimal supervision.


Major Responsibilities:



  • Efficiently conducts medication histories with patients in the emergency department and throughout the hospital.
  • Accurate and complete medication list gathering.
  • Responsible for working closely with Medication Reconciliation Pharmacist or ED Pharmacist to ensure accuracy and completeness of prior to admission medication lists obtained from patients.
  • Identifies and documents a patient's own medication supply for approved use during the patient's hospital stay.
  • Assists with requests for replacement doses, coordinates transfer of patient medications between nursing units, and monitors need for ongoing doses of continuous infusions
  • Assists in the management of restricted medications, non-formulary medications, and emergently needed medications
  • Updates and documents patient demographics to facilitate patient care.
  • Obtains patient-specific data to assist with departmental initiatives.
  • Reviews the patient's profile to maximize availability of medications and to decrease waste
  • Evaluates the patient's continuous infusions to maximize availability and decrease waste
  • Through profile review, identifies candidates for IV to enteral conversion
  • Under a pharmacist's supervision, educates patients, patient caregivers, nurses, and pharmacists on new products, pharmaceutical devices, and practice changes as assigned
  • Perform duties as a pharmacy tech as required including, but not limited to the following:

    • Maintains medication storage areas external to the pharmacy, including Pyxis Medstations
    • Prepackaging solid, oral liquid, and injectable medications including labeling and recording of required data and inventory logs.
    • Processing and accurately filling (or compounding) medication orders and performing functions required in an intravenous admixture program including initial calculation, preparation (including aseptic technique), and checking of work.
    • Reports and investigates medication errors.




Salary Range: Minimum $17.82/hour - Maximum $29.43/hour. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
