We do this through programs designed to combat homelessness and hunger, provide safe spaces for kids after school and on the weekends, achieve and maintain sobriety, and give hope to the hopeless. The Salvation Army North and Central Illinois Division provides these services to people in need. JOB SUMMARY This position will provide outreach, assessment, information, referrals and direct service to the homeless population who come to the Mobile Outreach Unit. PROGRAM/CLIENT ENVIRONMENT The backgrounds of the clients served by The Harbor Light Program will manifest a variety of inappropriate behaviors. When this occurs, employees are to respond within the context of the treatment environment of the site. Employee conduct (actions, dress, etc.) and interactions (conversations, written communications, etc.) with clients must also be within the treatment environment of the facility. ESSENTIAL DUTIES AND RESPONSIBILITIES
- Needs assessments are provided of the client's needs to determine appropriate internal and external support and services. Information and assistance are provided in finding solutions for clients who are considered domically homeless based on current needs. Additional referrals to an appropriate service or agency are made if applicable to client's needs.
- Direct assistance in handing out clothing vouchers, coat vouchers, implementing certified homeless letters and DMV vouchers, act as liaison between pharmacy companies and other medical clinics for medication retrieval and medical appointments. Direct advocacy for clients for federal and state funded programs and assistance with Medicaid and Link applications.
- Maintain a consistent caseload of 20 or more clients.
- Weekly supervision with Clinical Director
- Generate monthly statistical reports focusing on the needs and progress of clients in state funded programs. Case manager to input regular data entry
- Assist with CARF
- Facilitate weekly Life Skill Groups
- Collaborate with upper management, counselors, managers and directors regarding clients from our clinical program
- Participate in daily Review Board meetings to capture new intakes.
- Establish relationships with a variety of community agencies to find services appropriate for the needs of the clients.
- Interacts with the domically homeless population served by the Clinical treatment program to initiate and build relationships.
- Must be able to advocate, speak, read and write Spanish fluently.
- All other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES
- Education: Bachelors' Degree required, Masters preferred with a social work or counseling major, but other human service fields will be considered based on prior work experience
- CADC, Master in Social Work or Master in Counseling with a license is preferred.
- Experience: Minimum two years with case management experience working with homeless population and in substance abuse treatment setting
JOB LIMITATIONS
- This individual will only commit Army resources that have been allocated or approved.
- This individual will keep the Director of Clinical Programs informed on all critical issues relating to his/her area of responsibility.
- This individual will adhere to all Army policies and procedures in carrying out the responsibilities of this position.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- This position is required to do light to strenuous physical work. In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and PC.
- Must be able to walk up two flights of stairs and lift up to 50+lbs.
- The work environment for this position includes an office and outside environment with a low to high noise level.
- Must be able to work in all types of weather elements including severe conditions.
- Will work inside a mobile feeding truck that drives and makes frequent stops on a regular daily basis.
- Must be able to work with clients that have poor social skills, suffer from mental illness, and have substance abuse problems.

REPORTING RELATIONSHIPS This position reports to the Director of Clinical Programs. PERFORMANCE MEASURES This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy and completeness of accomplishing assigned goals.
