Join the dedicated Geriatrics team of providers at Corewell Health West providing care to complex patients in West Michigan. If you have a passion for helping patients and families manage complex, multi-dimensional challenges in a variety of settings, this is the opportunity for you! As an Advanced Practice Provider (APP), you will care for geriatric, frail, and medically complex patients in various primary living situations. While you will be part of a collaborative & multidisciplinary care team, the APP must also be comfortable seeing patients independently and enjoy autonomy in this role. This is a full-time position working five 8-hour days per week.
