Job Summary
The Quality Assurance Specialist II supports organizational quality and compliance efforts by reviewing documentation across levels of care and programs, monitoring incident reporting, conducting audits, and contributing to continuous quality improvement. The role ensures adherence to regulatory requirements, contract obligations, internal policies, and accreditation standards while partnering respectfully and constructively with staff to improve consistency, service quality, and operational effectiveness.
Education & Experience.
* High school diploma or equivalent required.
* Additional education or training related to quality assurance, health information, behavioral health, compliance, or healthcare administration is preferred.
* A bachelor's degree in Health Information Management, Health Administration, Public Health, Human Services, Behavioral Health Studies, Healthcare Compliance, or a closely related field is preferred but not required.
* Experience working in substance use disorder or mental health service settings.
* Experience conducting documentation review in a clinical, behavioral health, or healthcare environment.
* Experience applying ASAM criteria or levels of care in reviewing documentation.
* Experience working in a fast-paced environment with multiple competing deadlines.
* Experience using diagnostic and billing codes relevant to behavioral health documentation.
* Experience preparing summaries, compiling data, or producing reports that support quality or compliance activities.
* An equivalent combination of documented training, coursework, and relevant experience may be considered in place of a degree when it demonstrates the knowledge and competencies required for this role..

Essential Job Duties
* Reviews clinical and service level operational documentation to evaluate quality, completeness, compliance, and alignment with organizational requirements to support high quality and consistent service delivery.
* Tracks, reviews, and analyzes incident reports to identify patterns, support accurate reporting, and strengthen organizational safety practices.
* Partners with program staff, supervisors, and leadership to support clear and timely incident documentation and enhance understanding of reporting expectations.
* Conducts scheduled and targeted quality audits including chart reviews and program audits to assess compliance and identify opportunities for improvement.
* Processes subpoenas and court related documentation and supports witness preparation to ensure accurate and timely fulfillment of legal obligations.
* Administers patient and client satisfaction surveys and prepares summary results to inform service improvement efforts.
* Supports quality improvement initiatives by preparing data, tracking action items, and contributing to quality improvement activities.
* Serves as a member of the Safety Committee and produces reports to support organizational safety goals and compliance requirements.
* Participates in onboarding activities and presents quality and documentation requirements to ensure staff understand expectations.
* Completes assigned projects and additional responsibilities to support department operations and organizational quality objectives.
Knowledge, Skills, & Abilities
* Knowledge of clinical operations and workflows in behavioral health and managed care settings.
* Knowledge of quality improvement, compliance, and utilization management practices relevant to behavioral health documentation.
* Knowledge of ASAM criteria and documentation standards used in patient placement and level of care review.
* Skilled in reviewing documentation for accuracy, clarity, completeness, and compliance.
* Skilled in managing work tasks, meeting deadlines, and organizing workflow in a fast paced environment.
* Skilled in written and verbal communication including preparing clear summaries and reports.
* Skilled in using Microsoft Office applications and electronic information systems.
* Ability to work collaboratively and respectfully with staff across departments to support quality and compliance expectations.
* Ability to maintain confidentiality of sensitive information consistent with policy and regulatory requirements.
* Ability to manage multiple tasks at once and adapt to changing operational needs.
* Ability to apply strong attention to detail in reviewing documentation and preparing quality reports.
Other Eligibility Requirements
* Must complete a criminal history background check and receive authorization from the State of Oregon's Background Check Unit.
* Must not be excluded from participation in federal healthcare programs, including but not limited to listings on the Office of Inspector General (OIG) List of Excluded Individuals/Entities and the System for Award Management (SAM) exclusion list.
* Must be legally authorized to work in the United States without sponsorship.
Travel Requirement
* This position does not require regular business travel. Occasional overnight travel may be necessary for training, meetings, or other business needs.
