Overview
The Intake Liaison plays a central role in coordinating the referral-to-admission process for home health patients. This position ensures physician orders, agency policies, and regulatory standards are met while facilitating a smooth transition into care. The role combines administrative precision with patient-centered communication to support agency growth and operational excellence.

Key Responsibilities
* Coordinate the timely conversion of referrals to admissions, addressing barriers through collaboration with clinical, marketing, and operational teams.
* Serve as the primary communication link among patients, families, physicians, and agency staff to ensure continuity of care.
* Review referrals for appropriate home health orders and verify coverage from payer sources.
* Participate in interdisciplinary team meetings, in-service training, and quality improvement activities.
* Ensure adherence to agency policies, accreditation standards, and federal and state regulations.
* Protect patient confidentiality and uphold patient rights in all interactions.
* Prepare and submit timely reports on admissions, census, and other critical metrics.
* Support onboarding and mentoring of new staff as needed.
* Represent the agency positively in the community and maintain strong referral relationships.

Qualifications
* Associate degree in a related field preferred.
* Experience in a healthcare setting, preferably in home health or intake coordination.
* Strong communication and customer service skills.
* Proficiency in database management and inquiry systems.
* Ability to work collaboratively in a fast-paced environment.

Work Environment
The position operates primarily in an office setting, involving regular computer and phone use with occasional light lifting. Minimal travel within the service area may be required. This position does not involve tasks related to exposure to blood, body fluids, or OPIM.
