Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER This is full-time role primarily remote, with a hybrid structure. The position requires occasional in-person clinic visits (approximately twice per month) and periodic trips to the administrative office for tasks such as mailing outreach (around once every other week). The Quality Improvement Specialist is responsible for ensuring the organization and delivery of high-quality patient outreach and quality improvement initiatives. This includes developing and participating on GHC-SCW committees and ongoing collaboration with other members of the Quality and Population Health team. Participation includes recommending, assisting, and selecting in the design of programs, quality improvement initiatives, and outreach methods to help members inclusive of all product lines manage their preventive health and chronic illnesses. Outreach methods may include telephone calls, digital contacts, mailings, and arranging and scheduling appointments and other services. The incumbent performs the job responsibilities with a health equity framework and with independence in decision making. The incumbent will work closely with partnering organizations to ensure GHC-SCW's full membership is served. This includes but is not limited to Access Community Health Center and UW Health. The incumbent represents GHC-SCW at community events and provides and supports additional and administrative duties, as assigned. This position requires excellent written and verbal communication, documentation, computer, organization and prioritization skills. As well as strong analytical, critical thinking, and problem-solving skills. Primary direction, guidance and supervision is provided by the Clinical Quality Coordinator with additional accountabilities to the Quality & Population Health Director, as assigned.
