Part Time

The Social Worker must have obtained an MSW and meet the clinical license requirements for the state (ex. LCSW, LICSW)

The Social Worker will support DaVita Skilled Nursing Facility Dialysis (SNF) patients. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside, instead of being transported to an outpatient dialysis center.

As a Social Worker at DaVita, you'll be a part of a Team that values work-life balance and where your personal and professional growth is a top priority. DaVita has an open position for a Social Worker who will be a vital member of each patient's core care team.
