Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder. Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder, mental health diagnosis, or other medical conditions. Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome Provide continuity of care by meeting patients in various clinical and community settings including the following: home visits, emergency department visits, addiction treatment appointments, medical appointments, social service appointments, correctional visits, and therapeutic appointments. Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc. Perform outreach by meeting with patients in the community and act as allies and advocates for patient services Attend Addiction Medicine clinical and programmatic conferences and meetings as required. Engage in ongoing professional development, training and receipt of clinical supervision. Experience Required 1 year of relevant experience if candidate has Bachelor's degree in related field or 2 years of relevant experience if candidate has Associates degree or is a Certified Peer Recovery Specialist. 4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant. Education Requirements Bachelors' degree in a related field, plus 1 year of relevant experience working with patients with Substance Use Disorder OR Associates degree with 2 years of relevant experience OR Certified Peer Recovery Specialist designation plus 2 years relevant experience OR Certified Medical Assistant with 4 years of relevant experience OR 4 years of relevant/lived experience License/Certification Requirements Valid driver's license REQUIRED. Must possess own vehicle or reliable alternate means of transportation for use to outreach individuals served. Special Requirements Must be comfortable working with an underserved patient population and utilizing harm reduction models of care. Must be comfortable working with patients in the community, attending appointments with them in a variety of seeting and ongoing advocacy work. Excellent interpersonal and communications skills. Be willing to engage in cooperative, professional relationships with all members of the interprofessional healthcare team. Hourly Rate Min $24
Hourly Rate Max $40
