JOB DESCRIPTION:

SUMMARY OF POSITION

Performs a variety of complex clerical and other supportive duties requiring considerable independent judgment and action with minimal supervision. This role involves providing essential administrative support to the assessment team, ensuring smooth communication and efficient management of assessment processes. The assistant acts as a key contact for stakeholders, manages documentation, and coordinates assessment schedules to facilitate timely and effective service delivery.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Serving as the designated contact for stakeholders and collaterals to facilitate follow-up needs, questions, and communication, including closing the loop on referrals and coordinating with HIM department on record sharing
  • Completing assessment verification letters and follow-up correspondence
  • Managing follow up on required assessment documentation and barriers to completion
  • Ensuring timely communication with collateral contacts
  • Performing data entry, including manual forms for walk-in assessments
  • Conducting outreach to scheduled appointments to improve attendance rates
  • Coordinating contingency plans and collaborating with staff when assessments are full
  • Supports completion of administrative paperwork - either directly completing or coordinating with other staff for completion

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.
  • Ability to deal with problems involving a few concrete variables in standardized situations.
  • Intermediate skills and knowledge of Windows and Microsoft Office Products.
  • Strong organizational and time-management abilities
  • Ability to work collaboratively in a team environment

QUALIFICATIONS

Education Level

Associate's degree required.

With Director approval, equivalent experience may be considered in lieu of education.

Years of Experience

Six months to one year of related experience and/or training required.

Required Certification/Licensure

Must possess a valid driver's license, have unlimited access to personal transportation and meet the agency requirements for auto insurance.

Must also meet the agency requirements regarding use of agency vehicles in the transportation of clients.

Must be able to pass a DCFS background check, criminal background check to include fingerprinting, and nurse's aide registry check.

$18.20--$25.50
