Overview Now hiring a full time Patient Care Coordinator for Ohio Valley Home Health! This is an onsite, Monday-Friday 8am-4:30pm position. NO WEEKENDS or HOLIDAYS! Responsibilities The Patient Care Coordinator is responsible for the delivery of nursing and home health services to the patients served by the Agency, quality of nursing services, effective resource utilization and appropriateness of care. The Patient Care Coordinator provides direction and guidance to home health staff. The Patient Care Coordinator works collaboratively with other leaders in achieving Agency goals.
