Job Description

Manages, plans, organizes and evaluates the staff and activities of the referral center. In collaboration with the leadership, establishes, interprets and implements policies and procedures of the organization and Home Care Network. Interprets regulatory guidelines for admission to Home Care. Provides guidance to internal stakeholders and others requesting assistance or information regarding access to Home Care Services.

Job Responsibility




  • Provides leadership to Referrals team members and supervisors by communicating and guiding toward achieving department objectives.
  • Develops, communicates, and builds consensus for goals in alignment with the health system.
  • Manages and oversees the referral center staff and their activities; manages, hires, trains, disciplines, and evaluates the performance of staff; ensures performance appraisals are completed in a timely manner.
  • Collaborates with leadership concerning personnel problems and subsequent disciplinary actions; acts as a resource to professional staff regarding patient related issues.
  • Participates in the on-going functioning, growth and development of the network; obtains and collects data to measure productivity and prepare reports.
  • Reports to leadership issues related to planning services; identifies potential shifts in service care needs or health care trends impacting future planning.
  • Promotes awareness of the network and its services within the organization and to the general community; oversees Home Care Liaisons, as needed; interprets the Home Care Network's policies and procedures; serves as a participating member on the Network's professional committees.
  • Develops and implements high standards of quality care; ensures staff adherence to standards of quality, appropriateness and compliance with regulations.
  • Evaluates staff's knowledge base and schedules in-service programs to enhance their skill set; provides information to staff on relevant continuing education opportunities.
  • Reviews support documentation and other information related to billing third party payers; manages daily Referral Center operations.
  • Monitors the screening and evaluation of patient referrals; ensures calls are handled in accordance with procedures and service requirements; oversees intake staff in determining appropriateness of referral; resolves problem/complaint calls, when necessary, from patients, families, physicians and/or hospitals.

Job Qualification




  • Graduate from an accredited School of Nursing required.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
  • 6-8 years of relevant experience and 2-5 years of leadership / management experience, required.
