The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. We are deeply committed to our clients and are dedicated to implementing a comprehensive model of care. The Guidance Center (TGC) is currently seeking a Crisis Stabilization Case Manager. This individual provides case management services to adult clients in crisis in the context of a short-term Crisis Stabilization Clinic. This individual will coordinate communication and services to internal and external partners and provide direct care towards stabilization of mental health and substance use symptoms. Responsibilities
- Gather information about the client and refer to the appropriate resource to handle the problem such as a same day appointment with a licensed therapist, emergency room at local hospital, same day appointment at local office, welfare checks, diagnostic evaluation, etc.
Provide support to other providers both in and out of the office, as well as to other community entities dealing with a client in crisis and needing the assistance of crisis stabilization services.
Make follow-up calls to clients who fail to show for post crisis follow-up appointments or who may need extra contact to effectively cope with a current Crisis.
After a QMHP has completed an evaluation, assist in coordinating access to the appropriate level of care.
Collaborate with other team members and other outside providers in the coordination of appropriate services.
Help maintain the inner workings of the Crisis Stabilization Center, which includes but is not limited to, general care and support of clients, daily operations, and housekeeping of the unit.
Support other staff members as needed.
Assess, make decisions, and utilize deescalating skills in intense situations.
Maintain effective and cooperative working relationships with TGC staff as well as referral sources and community partners through accurate, timely, and professional communication.
Adapt to changing work priorities and a fast-paced environment while maintaining professionalism
Participate in training as directed by the immediate supervisor or TGC management staff.
Meet HIPAA standards for confidentiality of medical records
Qualifications:
- Bachelor's degree required. Degree in Social Work, Psychology, or Human Service field preferred
Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered
One year volunteer or paid experience working with individuals in the community.
