POSITION SUMMARY: The Medical Case Manager is responsible for assessing the needs of assigned patients caseload and identifying barriers to care, including comprehensive insurance, risk assessment, medical, adherence, and treatment needs; developing service plans based on those identified needs; facilitating referrals and access to social service benefits and other internal and external resources and other providers. Monitors patient progress and advocates on behalf of patients when they are unable to do so for themselves; documents in the electronic medical record and complies with all data entry, data integrity, and data tracking requirements for BMC and funding agencies contract monitoring reports. Position: Medical Case Manager Department: Infectious Disease Schedule: Full Time, 40 hours, Days JOB REQUIREMENTS EDUCATION: Master's Degree in social work, psychology, or related human services field (equivalent experience may substitute for degree). LCSW, LICSW or LMHC as appropriate, preferred. EXPERIENCE: Experience working with HIV/AIDS clients, and related issues, including co-morbidities. Effective verbal and written communication skills in English. Bilingual-bicultural is a plus. KNOWLEDGE AND SKILLS:
