Position Summary:
  • In partnership with the Social Worker assigned, this position oversees and coordinates all services for children, youth and young adults. Acts as a liaison to various departments, placing agencies, community resources and affiliated agencies.
Essential Duties:
  • Assist Social Workers with referrals, tracking of necessary documents required by contract.
  • Auditing the contractually required electronic portal system and maintaining communication with various treatment team members, ensuring necessary follow up.
  • Facilitates support groups, and training for assigned children and/or Resource Parents as required by program or contract.
  • Assists in providing child and family with a structured orientation to address policies and procedures as well as questions or concerns.
  • Provides relevant information to assist with appropriate treatment interventions and schedule consultations with treatment team, as needed.
  • Documents services provided to each child, youth, or young adult as well as maintain accurate logs for outside services.
  • Provides necessary transportation and escorts client to court appointments, medical health screenings and other services, as needed.

Salary Range:

$21.00-$27.30

Requirements

Minimum Qualifications:
  • High School Diploma or GED. Must be able to operate standard office equipment and be computer literate. Applicant should possess excellent verbal, written, analytical and problem-solving skills. Must be able to communicate effectively and follow both oral and written instructions.
Additional Requirements:
  • Must possess and maintain a valid, unrestricted California driver's license and current vehicle insurance.
  • Must be able to understand and follow policy and procedures.
  • Must be able to document services, incidents, and other required forms/documents.
Physical Requirements:
  • Frequent carrying, up to 25 pounds
  • Infrequent bending, stooping, twisting, turning, and squatting
  • Infrequent pushing/pulling up to 10 pounds and 25 feet
  • Infrequent lifting, floor to waist and floor above shoulders
  • Regularly sitting, 4 to 6 hours per day, depending on the schedule
  • Regularly standing, 2 to 4 hours per day, depending on the schedule
  • Regular use of fine motor skills for such activities as writing and entering data into various computer systems
  • Normal or corrected vision needed for driving and documentation
