Responsibilities

Palo Verde Behavioral Health is seeking a Step-Down Outpatient Coordinator/Coordinator Facility Transition to join our team!

The Step-Down Outpatient Coordinator/Coordinator Facility Transition is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Outpatient Coordinator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. The Step Down Outpatient Coordinator frequently supports the Outpatient Programming and Therapists as directed by Outpatient Leadership.

Qualifications

QUALIFICATIONS

Education: Some college in mental health/behavioral sciences. Preferred: Bachelor's degree from an accredited college or university in social work, psychology, mental health or a related field

Experience: A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting

Additional Requirements: CPR and HWC training as outlined in New Hire Policy must be completed before providing direct care services.

Must be able to obtain and maintain an Arizona Class 1 Fingerprint Clearance Card.

STANDARDS OF PERFORMANCE

THE ESSENTIAL JOB FUNCTIONS ARE CATEGORIZED UNDER THE JCAHO STANDARDS OF PERFORMANCE.

Program Planning and Treatment

  1. Consults with the Social Services, Utilization Management and Outpatient on patient eligibility
  2. Maintains current awareness of Outpatient programming
  3. Maintains Midas data collection process
  4. Identifies and communicates data driven trends in the step-down process
  5. Assist in the development of strategies, goals and objectives related to the step-down process
  6. Consults with facility management, ensuring awareness of step-down trends
  7. Supports Outpatient Programming and Outpatient Therapists as directed

Continuous Quality Improvement

  1. Ensure the Step-Down process is maintained to provide continuity of care
  2. Evaluate trends and establish goals as it pertains to the step-down process
  3. Ensure the collection and entry of data as it pertains to patient eligibility
  4. Review & Report on data and processes routinely in Quality/PI meetings
