Position Summary/ Department Summary:
Provides clinical expertise in school systems, IEP process and in-depth knowledge in community-based resources specific to the identified patient population. Consultation to Clinical Team both inpatient and outpatient. Providing culturally informed content empathically with a diversity, equity and inclusion lenses. May serve as lead to level I Patient & Family Educator's.

Key Responsibilities:

  • Provides clinical consultations for teaching intervention to patients and families who come to the PFE's assigned Program, using skills in the evaluation of patient and family educational and home support needs, with an understanding and appreciation of families with complex demands.
  • Establishes rapport, supportive and on-going relationships with patients and their families from a variety of cultural, ethnic and religious backgrounds.
  • Assists with the development and management of individual, family, and community health education programs.
  • Monitors healthcare technology using REDCap, ensures that health education programs are current and monitors, assesses, and reviews educational programs for effectiveness.
  • Develops materials to be implemented into health education programs that satisfy the needs and interests of plan enrollees, employers, and providers.
  • Responsible for consulting directly with individual patients and their families to help them understand the medical care that they are being given as well as to connect them with outside resources and support groups.
  • Leads a team that identifies, develops, integrates and rolls out resources such as guides, brochures and informational packets
  • Train/lead new staff.

Minimum Qualifications

Education:

  • Bachelor's degree in education, psychology or related field required
  • 2 - 4 years professional experience with pediatric patients and families required
  • Proficient in excel, comfortable learning new computer applications preferred
  • Proficient organizational skills required
