HealthPartners is currently hiring for an RN - Supervisor, Hospice & Palliative Care. This position oversees the daily operations of the Hospice & Palliative Care program, ensuring smooth and efficient functioning. The supervisor provides essential guidance and closely monitors the performance of the Hospice & Palliative Care staff. Collaborating with the team, this individual promotes and develops optimal palliative care plans for patients and their families. The supervisor reports directly to a Hospice Senior Manager and collaborates with other hospice supervisors across the organization. They lead a dedicated team of 20-25 nurses, ensuring cohesive and effective management. Required Qualifications:
- Registered Nurse with current license in the state of MN.
- Clinical Hospice/Palliative or related supervisory experience.
- Demonstrated strong interdisciplinary and team-building skills.
- Current driver's license and a vehicle.
- Experience working with electronic health records.
- General computer and electronic literacy.
- Must be able to read, write and communicate clearly in English.
Preferred Qualifications:
- BS or BA in health-related field.
- Three (3) years of previous supervisory experience including hiring, orientation, and evaluation of personnel.
- Three (3) years in hospice or community-based delivery systems.
- Five (5) or more years of clinical experience.
- Experience in a Medicare certified hospice agency.
- Board Certification (Certified Hospice & Palliative Care Nurse or other appropriate certification)
Hours/Location:
- M-F; Days
- This is a hybrid position, requiring onsite presence at least two days per week.
- Occasional travel will be necessary to conduct joint visits with direct reports in the community.
- This position will be part of a rotating leadership on-call schedule.
Responsibilities:
- Establishes and Maintains High Performance Work Unit.
- Responsible for the supervision of staff, including having the authority to hire, transfer, lay off, promote, discipline and discharge, train, reward, and review performance of employees. Ensures compliance to organizational and departmental policies and procedures.
- Ensures that employees meet requirements of clinical/profession-based competency, operational/regulatory standards, and respectful behaviors. Performs joint home visits, audits documentation routinely and regularly.
- Supervises within employment law, regulations, and HealthPartners' policies and collective bargaining agreements (if applicable).
- Attends Interdisciplinary Group (IDG) and other meetings as assigned that support team functions.
- Provides regular consultation to the Hospice Intake and Staffing departments regarding referrals, scheduling of visits and admissions, and other.
