MGR-CASE MANAGEMENT

General Purpose of Job:

Provides leadership and direction for the Case Management area in the department of Patient Support Services. By translating the Hospital and Health System's vision and strategies into practice, the Manager contributes to high quality, compassionate, cost-effective services provided to patients.

The Manager works in collaboration with Administrative staff, Directors, Managers, Physicians and other Sparrow Caregivers to achieve care coordination and integration of services approach for the patient's experience of care.

• Participates inefforts to support and advance the Health System's mission, vision,strategies and goals.
• Provides leadership tothe Case Management area of Patient Support Services Department (PSS).
• Works in conjunctionwith executive and clinical leadership, administration, other staff, andexternal agencies, and, as appropriate, will advise regarding current andprojected trends, areas of deficiency and significant activities relatedto the functions of case management.
• Facilitates andfosters the development of a collaborative relationship between case management, nursing, other departments and the medical staff.
• Represents the Hospital and Health System to the community, educational institutions,professional organizations, payers and others.
• Ensures fullcompliance with regulatory and accrediting agencies.
• Develops andimplements short and long term goals for case management that reflect theHealth Systems strategic vision and goals.
• Selects, orients, mentors and evaluates department staff members.
• Maintains a high levelof staff morale, professionalism, efficient and effective performance, motivation and pride in the services performed for patients and the HealthSystem.
• In conjunction with theDepartment Director, appropriately plans for and projects the annual casemanagement financial plan. Monitors the results and advises leadership whenthere is a need to adjust the plan.
• Monitors customersatisfaction, performance improvement and corporate compliance indicators, and key indicators of effectiveness and efficiency to ensure optimal performance.
• Participates inestablishing policies and procedures, position descriptions, staffingneeds, staff scheduling, and productivity.
• Assists in thedevelopment and implementation of special projects.
• Supervise processesand personnel directly involved in securing authorization and financialfunding from payers for discharge planning needs.
