Description Position: Community Health Worker Location: Rockport, ME Schedule: Full-Time Overview: Join the MaineHealth Community Health Department as a Community Health Worker and play a vital role in helping patients access resources and services, support disease self-management, and coordinate culturally appropriate care. This position combines health and wellness coaching with care management and transition support to provide a holistic approach to patient well-being. Position Highlights:
- Work within a team-based, patient-centered care management model.
- Support patients and their families in understanding and managing medical conditions.
- Provide care coordination and support for chronic disease management.
- Assist with complex care planning, including medication review and education.
- Help patients complete advanced care directives, insurance paperwork, MaineCare applications, and disability forms.
- Address social determinants of health such as food insecurity, housing instability, transportation barriers, medication access, and health literacy.
What You'll Do: As a Community Health Worker, you will make a meaningful impact by identifying and addressing barriers to care, including affordability, transportation, housing, food security, and literacy. You'll interact with patients through phone, virtual visits, face-to-face meetings, and community or in-home visits-tailored to meet individual needs. You'll collaborate with an interdisciplinary team to bridge clinical care with community resources, helping to improve health outcomes and enhance the patient experience. Please note: This role does not involve providing hands-on medical care or patient transportation. - Education: High school diploma or GED preferred.
- License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
- Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
- Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
- Additional Skills/Requirements Preferred: Basic computer skills preferred.
