CARE COORDINATOR - Full Time Bellingham, WA What you will be performing The Care Coordinator is responsible for ensuring an integrated, whole person health approach to address the health and well-being of individuals served. Services delivered by the Care Coordinator includes but is not limited to case management; treatment planning; resource identification and assistance; identification, development, and collaboration with natural supports and providers. Serves as a treatment team member and provides back up to other team members as needed.
- Establishes a treatment plan and goals with client and evaluates progress throughout treatment.
- Works collaboratively and coordinates with outpatient psychiatric/counseling services, emergency services, other treatment team members, etc.
- Coordinates services with community agencies (e.g. Light House Mission, Lake Whatcom Center, Road2Home, various Teams, etc.) and works in context of a treatment team to connect client with needed support for after discharge from Triage, as needed and appropriate.
- Makes appropriate referrals for auxiliary services such as drug and alcohol services when clinically indicated.
- Coordinates care with primary care and collaborating professionals from other systems as well as natural supports as appropriate.
- Conducts follow-up phone calls to check on client progress after discharge.
- Attends multi-system community-based meetings in local area as needed for coordination of care.
- Participates in regularly scheduled supervision and team meetings.
- Complies with all accurate and appropriate charting, coding, and billing procedures as required by the regulatory organizations.
- Documents delivery of services in the medical record. Completes all paperwork to agency standard and within required timelines.
- Meets or exceeds key performance indicator expectations and/or response time expectations established by the agency.
- Remains compliant with required trainings and certifications.
- Behaves in an ethical and professional manner consistent with agency policy, state law, and licensing entities if applicable.
- Understands and practices safekeeping of client protected health information per Compass Health's HIPAA and other agency policies and procedures.
- Demonstrates respect for diversity and a commitment to developing multicultural competency and sensitivity.

What you will be carrying along
- BA/BS/BSW Degree in a Behavioral Science related field.
- Experience in providing care coordination or case management services preferred.
- Demonstrated knowledge and experience with Evidence-Based Practices, or interest and ability to learn.
- Individuals who do not already hold a license or associate license through the Washington State Department of Health must submit Agency Affiliated Counselor application upon hire and be approved by the DOH.
- Current First Aid & CPR certification, or ability to obtain within 90 days of hire.
- Valid Washington State Driver's License including vehicle and appropriate insurance.
- Bilingual/bicultural encouraged to apply.
