POSITION DESCRIPTION

POSITION TITLE: SSVF HUD-VASH Liaison Manager

(Coordinated Case Management - SSVF CCM)

LOCATION/DEPT: The Salvation Army Veterans Services/Social Services Dept.

REPORTS to (TITLE): Director of Veteran Services

FLSA CATEGORY: Exempt

SALARY RANGE: Grant Funded, $52,083 - $72,928 Annually

OUTCOMES:

NCI SSVF HUD-VASH Liaison Manager is a member of the Supportive Services for Veteran Families (SSVF) Team. The goal of the SSVF HUD-VASH Liaison Manager is to provide time-limited Collaborative Case Management to expedite housing placement for homeless Veterans. The SSVF HUD-VASH Liaison Manager provides housing navigation and case management services to participants co-enrolled in SSVF (Supportive Services for Veteran Families) and HUD-VASH. The SSVF HUD-VASH Liaison Manager will work with referred Veterans and their families to assess housing resources, needs, barriers, ad make an appropriate housing plan toward housing stability and permanence. The SSVF HUD-VASH Liaison Manager will provide ongoing case management to participants with pending HUD (Housing & Urban Development)-VASH (Veteran Affairs Supportive Housing) housing vouchers. The SSVF HUD-VASH Liaison Manager will work closely with the Department of Veteran Affairs to ensure compliance with the SSVF and HUD-VASH guidelines. The SSVF HUD-VASH Liaison Manager will engage in efforts to increase the SSVF presence at each CoC.

ESSENTIAL DUTIES:

Case Management (60%)



  • The SSVF HUD-VASH Liaison Manager will be the SSVF Point of Contact (POC) who will be the primary POC for the Veteran and HUD-VASH staff.
  • Communicates with the local PHA regarding coordination of the PHA issuance and utilization of the HUD-VASH vouchers.
  • Communicates with the Continuum of Cares (CoCs) to ensure each local CoC is aware of HUD-VASH/SSVF CCM so any local Coordinated Entry processes continue to be followed or adapted. Is responsible for reviewing the agreements between Property Managers, PHAs and the VA to establish appropriate referral sites for placement.
  • Work closely with the VA medical center's homeless programs to identify, assess, refer and admit Veterans into SSVF for co-enrollment.
  • Works with the HUD-VASH team to assist the Veteran in obtaining their HUD-VASH voucher application, completing the necessary pre-housing paperwork, obtaining required documents and submitting completed voucher application packets.
  • Works with the Veteran on searching for housing, including the rental application process, expected communication with landlords, and securing move-in assistance as needed.
  • Ensures program criteria, referrals, and procedures are maintained for HUD-VASH, SSVF and program participants.
  • Works with the HUD-VASH team in the lease up and transition from SSVF Temporary Financial Assistance (TFA) with obtaining the Request for Tenancy Approval from the PHA.
  • Provides ongoing case management for up to one year after move-in to ensure a seamless transition from SSVF into the most appropriate HUD-VASH case management stage.
  • Facilitates person-centered planning to determine vocational goals, recommend appropriate services to achieve goals and oversee the program and objectives agreed upon.
  • Locate landlords willing to lease to clients with housing barriers.
  • Understands State and Federal Landlord-Tenant laws.
  • Assists the SSVF Region 3 Supervisor with the processing of all HUD-VASH Referral Packets.
  • Coordinates/participates in CoC meetings, By-Name-List Meetings, Prescreen/VA Case Conferencing Meetings.
  • Assists Veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services.
  • Maintains a strong understanding of the various Salvation Army programs offered to provide resources to Veterans.



Administrative (20%)



  • Participates in the quality assurance activities regarding the SSVF program (CARF). Tracks and reports unmet needs of participants and their families.
  • Maintain complete and accurate electronic client records that includes the assessment, eligibility, intake, case plan, documentation of progress, challenges, accomplishments, discharge plan and all follow up contact.
  • Responsible for data collection, administrative record keeping and performance measurements of participants to ensure documentation is current, accurate and in compliance.
  • Provide electronic detailed case note records.
  • Provides clear documentation for HUD-VASH enrollment.
  • Assists in the documentation regarding the effectiveness of service delivery using consumer, case management and community resource feedback.
  • Participates in other program and outcome evaluation activities.
  • Provides ideas for programmatic specific to educational needs of participants. Conducts programs as needed.
  • Assists in the overall monthly statistical data collection with regard to the SSVF program and submits to the Director of Veteran Services/SSVF Program Manager.
  • Maintain compliance in The Salvation Army's "Safe from Harm" Certification. (required)
  • Complies with all applicable training requirements, company safety, personnel and operational policies and procedures.
  • Participates in training and workshop program as requested by the Director of Veteran Services/SSVF Program Manager.
  • Provides oversight of the development of CCM processes and potential expansion of services throughout the catchment area.
  • Provide supervision of future SSVF CCM case managers as the need develops.
  • Perform other duties as assigned as related to the SSVF program.



Collaboration (20%)



  • Attends community meetings and Coordinated Entry By-Name-List reviews.
  • Attends community Veteran events such as Stand Down, Veteran Resource Fairs, etc.
  • Communicates effectively with VA HUD-VASH and Salvation Army personnel.
  • Attends weekly, monthly meetings and annual training events.
  • Attends regularly scheduled staffing meetings with the HUD-VASH/HCHV team from the VA medical centers.
  • Works in partnership with other SSVF Case Managers, Intake Specialists and support staff Workers and participates in regular case consultation.


OCCASIONAL OVERNIGHT AND WEEKEND TRAVEL

Involves occasional driving to offsite meetings/trainings and conducting occasional in-person meetings with program participants.



  • Occasionally work weekends to accommodate event schedules, or other organizational needs. Flexibility in scheduling is essential to meet deadlines and ensure successful event execution.
  • Must have reliable transportation, valid driver's license and auto insurance.


EDUCATION/EXPERIENCE



  • Possesses a minimum of a Master's degree in related field and experience with direct service to Veterans and/or marginalized populations, or a person with lived experience.
  • Advanced degree in social work, counseling or human services is required.
  • Strong knowledge of the HUD-VASH program.
  • An in-depth knowledge of VA programs and guidelines.
  • Previous experience with social services and/or homeless programs.
  • Experience working with Veterans and families.
  • Valid Driver's license/MVR clearance.
  • Veteran status preferred.

POSITION LIMITATIONS (What this position must do)



  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Director of Veteran Services informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.
  • This individual must receive successful completion of background screening as well as employment eligibility verification.



PHYSICAL DEMANDS/WORK ENVIRONMENT



  • This position is required to do light physical work.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
  • Must be able to occasionally lift or move up to 40 lbs.
  • The work environment for this position normally includes an office environment with a low to moderate noise level.
  • Must be able to travel to multiple sites.
