Position Purpose







The Care Coordination Assistant facilitates communication and coordination of health care services across the continuum, in a cost-effective manner that utilizes available resources and promotes positive outcomes. Under the direction of the Director of Nursing, the incumbent follows up on referrals from outside the Renown Health System and on ECIN, and implements the arrangements for post-acute services. Communications with internal staff, patient/caregiver and community providers must be accurate and timely.









Nature and Scope







The Care Coordination Assistant is accountable for the timely and accurate coordination of communication to facilitate implementation of the patient admission/discharge plan.

The incumbent maintains knowledge of community resources and serves as a liaison to providers, participates in the identification of potential admits and maintains documentation to meet facility standards.

The Care Coordination Assistant acquires and maintains knowledge and competencies related to the expectations of their position. Practice is aligned with the mission, vision and goals of the integrated health network. She/he participates in Quality Improvement initiatives.

This position provides clerical and support services.

It is common to encounter potential hazards in the healthcare environment. Some of these hazards could include, but are not limited to: Radiation, Toxic Chemicals, Biological Hazards, Heat, Noise, Dust and Stress. Renown Health has a Safety Management Program in place addressing these issues.

This position may provide patient care.

Education:



Must have working-level knowledge of the English language, including reading, writing and speaking English. Understanding of medical terminology preferred.



Experience:



Two years experience in the medical field required (preferred for cost center 200769)



License(s):



Certified Nurse Assistant license preferred but not required.



Certification(s):





Computer / Typing:



Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. Computer skills necessary to access the ECIN, Epic and the Achieve Systems.
