If you enjoy working with individuals during some of the most vulnerable times of their lives this may be just the opportunity you've been seeking.This individual will be part of the Permanent Supportive Housing team and works directly with members who are enrolled in the program with serious mental illness (SMI) or co-occurring mental health issues and substance abuse, to transition to and maintain stable housing. You will need to be professional, friendly, a self-starter, organized, and compassionate.

Behavioral Health Technician works 1-1 with persons with SMI designation that are living in Permanent Supportive Housing. The goal is to have the member maintain stable housing. Having full or partial resolution of the presenting issues. Psychiatric stabilization; symptom management and reduction; prevention of relapse. PermanentSupportive Housing wants to boost the members power to choose their own living arrangements and get services that are flexible based upon the support they need at any given time.This position requires skills for engagement, building relationship and coordinating care.

Terros Health is hiring a BehavioralHealth Technicianfor our Bridging the Gap (BTG) Complex Care Services - Permanent Supportive Housing Program, in Phoenix, AZ.

Full-Time

Schedule: Monday - Friday 8am - 4pm or 9am - 5pm

Behavioral Health TechnicianDuties Include:



  • Provides direct face to face community-based services to assist tenants to maintain their housing. Providing PSH services in both scattered-site and single-site Yucca/Vista (Yucca and Vista) sites assisting tenants to maintain their housing. If 24-hour 1-1 service, maintains clients in line of sight at all times. Documents all services provided including non-billable services.
  • Delivers outreach for the purpose of engaging tenants into PSH services. Orients potential tenants to the PSH program.
  • Arranges transportation for tenants as appropriate and as medically necessary, and determined by the PSH team, in personal vehicle or in Terros Health provided vehicle, when available.
  • Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a tenant's family, behavioral and general medical providers, community resources, and other involved supports.
  • Assists tenants with connection to physical health services, including Primary Care services and medical appointment follow up.
  • Provides direct care wellness management and recovery strategies using strength-based engagement and psycho-social/educational /rehabilitation services/groups as needed. Ensures linkage into community services and resources.
  • Actively engages tenants into services by utilizing motivational interviewing techniques creating an alliance that will support their success in reaching their treatment plan goals.
  • Employs therapeutic interventions for life skill development, participating in meaningful activities and daily living skills appropriate for identified goals on treatment plan including assertive outreach and engagement strategies.

Qualifications

  • High School Diploma or GED, as well as a combination of behavioral health education and work experience totaling a minimum of three (3) years; or an Associate's degree in any field, plus two years of work experience in a behavioral health service delivery working with Serious Mental Illness (SMI).
  • Customer service skills, excellent written and verbal communication skills.
  • Ability to work independently, with flexibility, and within prescribed deadlines
  • Competency in Word, Excel, Outlook and Nextgen.
  • Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health's driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.
