Inova Health System's Fairfax Hospital is looking for a Post-Transplant Coordinator - Infusion Nurse to join our team supporting Abdominal Transplants. This role is full-time, M-F, 8:30am-4:30pm (No weekends, on-call responsibilities are 3 nights per month & 1 weekend every 3 months). This individual will help bridge gap of resources/relocating patient for infusions post-transplant, within the Transplant department. Post-Transplant Coordinator - Infusion Nurse (RN) Job Responsibilities:
- Provides infusion for post transplant patients
- Support Post Transplant Coordinator when available
- Continually assesses patient needs and dynamically collaborates with the health-care team to address actual and potential patient care concerns and needs
- Completes documentation of the plan/orders/outcome related to the pathway according to policy.
- Delegates patient care activities appropriately. Responsible for aspects of care/communication related to assigned caseload of patients.
- Educates patient, family, and care providers. Plans, implements, and evaluates formal and informal education regarding all aspects of care for the patient, family and caregivers and documents appropriately.
- Actively participates in program quality assessment and performance improvement initiatives including committees or teams.
- Participates in quality improvement initiatives and assists with process and protocol development/revision based on evidence based/best practice findings.
- Collaborates effectively with the interdisciplinary team and leverages the expertise of the team to achieve patient outcomes.
- Ensures improvements in practice by assuming responsibility for continuing education including obtaining and maintaining professional certification.
- Performs other duties as assigned.
Post-Transplant Coordinator - Infusion Nurse (RN) Additional Requirements:
- Work schedule: full-time, M-F, 8:30am-4:30pm (No weekends, on-call responsibilities are 3 nights per month & 1 weekend every 3 months)
- Education: BSN
- Experience: 3 years as a RN
- Certifications: BLS from the American Heart Association
- Licensure: Licensed in the Commonwealth of Virginia as a Registered Nurse
Post-Transplant Coordinator - Infusion Nurse (RN) Preferred Qualifications:
- Preferably recent, hands-on bedside experience in inpatient, acute care, and/or med-surg settings
- Experience with starting IVs, administering infusion, using infusion pumps
- ACLS preferred - if not upon hire, must obtain within training period
