Job Description: Job Duties: The transitions of care clinical pharmacist will be responsible for helping ensure patients move safely through the inpatient and outpatient settings (admission and discharge ordering services). The pharmacists will work in conjunction with medication history technicians to prepare medication reconciliation on behalf of the provider. The transitions of care pharmacist will be integrated into the hospital's outpatient pharmacy and will be responsible for facilitating the quality of care the patient receives through direct and indirect patient interactions. Qualifications: Graduation from an accredited College of Pharmacy- PharmD. Current Indiana Pharmacist License or Eligible for Indiana Pharmacist License. BLS for Healthcare Providers required. 1+ years of Pharmacist experience required.
