Job Overview:

The Clinician oversees daily operations of the Hospital Acquired Delirium Prevention Program. This includes screening and enrolling patients into the program, orienting patients and families to the program and assigning appropriate intervention protocols, developing and overseeing implementation of appropriate staff and Intervention Assistant assignments for patients, assuming direct responsibility for the Orientation Protocol, Therapeutic Activities Protocol, Early Mobilization and Sleep Enhancement Protocol, assisting with recruitment, screening and placement of program volunteers, and training and retention of program volunteers.

Qualifications:

Required:

  • Bachelors degree in Human Services or Healthcare related field
  • One to three years of working in a related position
  • Bilingual in English and Spanish
  • Program development and training skills

Preferred:

  • Experience working with elderly individuals

Scheduling Requirements:

  • Shift working, 8:00 4:00pm each day
  • Monday-Friday
  • FT

Essential Functions:

  • Participate in daily unit-based Multidisciplinary Care Coordination Rounds and review CAM scoring for program eligible patients
  • Daily screening and enrollment of appropriate patients into program
  • Screening procedure includes medical record review, patient and/or family interview and nurse interview
  • Orient patients and families to the program
  • Develop and initiate individualized care plans using predetermined intervention protocols
  • Provide appropriate equipment
  • Update care plan as needed based on patients' needs and outcomes
  • Notify nursing staff and other appropriate staff of enrollment status
  • Implement intervention protocols by assigning patient-appropriate activities to the volunteers
  • Perform regular evaluations of all patients to track progress, respond to patient care needs, and update care plan
  • Monitor and record adherence of all protocols and program outcome variables
  • Establish and address reasons for non-adherence and implement changes to improve services
  • Distribute, collect and collate results of Patient Satisfaction Surveys
  • Provide IA with appropriate patient care assignments each shift
  • Provide routine supervision and support of IA, with ongoing feedback
  • Coordinate and direct IA Training program
  • Perform quarterly Quality Assurance measurements on IA interventions to assure interventions are correctly and consistently implemented
  • Coordinate IA schedule, arranging optimal patient coverage
  • Coordinate with Case Management and Social Work staff regarding patient's possible barriers to safe discharge
  • When appropriate, educate patients and families regarding community resources available
  • Provide community education programs about prevention of hospital-acquired delirium and the program at venue appropriate locations
  • Record and monitor adherence of all interventions
  • Generate progress reports as needed
  • Purchase and secure all program equipment
  • Maintain tracking records of all equipment while in patient use
  • Maintain cleanliness and working condition of all equipment
  • Document expenditures for supplies and activities
  • Collaborate with the Volunteer Services Department to establish a training
