Position Summary

The Cardiac Procedural Nurse Program Coordinator provides care coordination throughout the continuum to address barriers to care and improve timeliness and quality of services, by coordinating complex processes, developing nursing plans, and working closely with clinic leadership on process and quality improvement initiatives. Through therapeutic relationships with patients, families, self, and colleagues, the CPNC Coordinator functions as an interdisciplinary team member who provides quality patient care and fulfills department and organizational goals.

Duties & Responsibilities:
  • Serves as the primary clinical resource liaison for Cath Lab patients served at UI Health for all phases of care, including conferences, documentation, and treatment authorizations.
  • Provides comprehensive navigation services to patients being served by the UI Health Cath Lab, acting as a consistent point of contact, care coordinator, educator, and advocate throughout the care continuum.
  • Develops professional relationships with patients and serves as their consistent point of contact to coordinate and expedite their care.
  • Facilitates communication among specialties and patients including but not limited to scheduling appointments and diagnostics testing, Pre/post procedural calls, coordinating internal and external referrals, expediting imaging, arranging consults and procedural clearances, and ensuring outside records are available in time for scheduled services.
  • Participates in assigned Complex Family Planning conferences, documents discussion and decisions, implements and follows up on decisions made.
  • Verifies orders and coordinates scheduling of such orders received from physicians, physician assistants, and/or advance practice nurses and documents accurately in the electronic medical record.
  • Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide quality and efficient patient care coordination as measured by established nursing quality standards and other governing/accrediting health care bodies.
  • Collaborates with other nurses to identify, promote, and provide positive learning experiences for staff and students to provide mentorship.
  • Serves as an educator to the community and partners via community outreach at UI Health and updates education as needed.
  • Supports nursing and multidisciplinary research and quality improvement efforts conducted on the unit and UI Health.
  • Ensures procedural coding is accurate, and Cath/PCI registry metrics monitored.
  • Clinical duties as assigned.
  • Other duties as assigned.
  • Perform other related duties and participate in special projects as assigned.


Qualifications:

Minimum Qualifications

  • Graduate of an accredited nursing program required.
  • Current Illinois license as a Registered Nurse required.
  • BSN required. Master's degree in nursing or related field preferred.
  • Minimal of (3) years of experience in the area of specialization (Cath Lab and cardiology).
  • Exceptional organization skills, including attention to detail and responsiveness is required.
  • Willingness to learn to voice dictation required.
  • Familiarity with reading cardiology reports and reporting systems.
  • Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, families, and health care providers.
  • Strong project management aptitude with experience in planning, facilitating, and organizing improvement teams preferred. This position prefers experience in program development from early concepts to full implementation.
  • Professional confidence is required as role is largely independent and self-sustaining with key involvement as a process and change leader.
  • Self-directed and works well in groups/teams exercising sound clinical judgment.
  • Intermediate computer skills with the ability to become proficient in Electronic Health Record. Working knowledge of Epic Electronic Healthcare Record (EHR) preferred.
  • Familiarity with Microsoft Excel, PowerPoint, Word and Outlook
